10. Health, Information & Release Forms
Returning the All Programs Document found on this website does not enroll
you in any of our programs! Please do not send this form unless you have
confirmation from Elderhostel Headquarters in Boston of your enrollment.
Once you are enrolled, please print and fill out the All Programs Document completely (both pages) and return
IMMEDIATELY to us. MOST IMPORTANT, we need this information for nametags,
hotel and room assignments and transportation requirements, which are done
weeks before a program. Elderhostel Headquarters has a new security
policy that prevents us from printing your city on the nametags and
participation lists. |
